Community Access & Social Support is about helping people stay connected to their community, maintain relationships, and take part in everyday life. At Smyth Support, we provide flexible, person-centred support that builds confidence, independence, and wellbeing — whether that’s getting out and about or enjoying meaningful activities at home.
This service is available to NDIS participants, aged care clients, and private clients, and is tailored to individual goals, interests, and support needs.

Support is always guided by the client’s preferences, abilities, and agreed goals. Depending on needs, this service may include:
Social outings and community participation
Assistance attending appointments, social groups, events, cafés, libraries, or local activities.
Companionship and conversation
One-on-one support focused on reducing isolation, building confidence, and maintaining social connections.
Support with hobbies and interests
Assistance to engage in personal interests such as walking, shopping, fitness, arts, games, or other recreational activities.
Skill development and independence building
Encouraging daily living skills, decision-making, communication, and confidence in community settings.
Transport support
Safe transport to and from community activities, as agreed and in line with service arrangements.
Support may be provided in the community, at home, or a combination of both, depending on what best suits the client.

We take the time to understand what matters to each person before support begins. This includes:
Identifying personal goals, interests, and routines
Understanding support needs, risks, and preferences
Providing consistent, reliable support workers
Adjusting support as goals or circumstances change
All support is delivered with consent, respect, and clear communication, in line with NDIS Practice Standards and Aged Care Quality Standards.

NDIS plans (where community participation or social support is funded)
My Aged Care services, depending on eligibility and approved supports
Private, self-funded arrangements
We’re happy to talk through funding options and help you understand what’s available.

Hold required clearances and checks
Are trained in safe work practices and duty of care
Follow individual support plans and risk assessments
Provide professional, respectful, and reliable support
